DOWNLOAD MEMBERSHIP FORM & PAY VIA CHECK-  with this option, you will download a PDF document to your computer.  Before you print & fill it out, be sure to select your membership option from the dropdown menu.   Print, fill in information, write check to OSC for necessary amount, and mail to provided address.
